Your Information. Your Rights. Our Responsibilities.
This notice describes how medical information about you may be used and disclosed, how you can obtain access to this information, and your rights regarding your health information.
Please review it carefully.
This Notice applies to protected health information maintained by Sprouts Health, LLC in connection with healthcare services provided by Zhanna Tarjeft, FNP-BC, FMCP-M.
HIPAA requires covered healthcare providers to explain how protected health information may be used and disclosed, describe patients’ rights, explain the provider’s legal duties, identify a privacy contact, and provide an effective date.
Your Rights
When it comes to your health information, you have certain rights. Sprouts Health is responsible for helping you exercise these rights.
Get a Copy of Your Medical Record
You may ask to see or obtain an electronic or paper copy of your medical record and other health information Sprouts Health maintains about you.
We will generally provide a copy or summary within the time required by applicable law.
A reasonable, cost-based fee may be charged where permitted by law.
Ask Us to Correct Your Medical Record
You may ask us to correct health information that you believe is incorrect or incomplete.
We may deny your request in certain circumstances, but we will explain the reason in writing when required.
Request Confidential Communications
You may ask us to contact you in a specific way, such as by telephone, email, or another address.
We will accommodate reasonable requests when required by law.
Ask Us to Limit What We Use or Share
You may ask us not to use or share certain health information for treatment, payment, or healthcare operations.
We are not always required to agree to your request.
If you pay for a healthcare service or item entirely out of pocket, you may ask us not to disclose information about that service to your health plan for purposes of payment or healthcare operations, unless disclosure is required by law.
Get a List of Certain Disclosures
You may ask for a list, called an accounting of disclosures, showing certain disclosures of your health information made during the applicable period before your request.
The accounting does not include every type of disclosure, such as many disclosures made for treatment, payment, or healthcare operations.
Get a Copy of This Notice
You may request a paper or electronic copy of this Notice at any time.

Choose Someone to Act for You
If you have given someone medical power of attorney or if someone is legally authorized to act for you, that person may exercise your rights and make certain choices about your health information.
We may verify that the person has appropriate authority before acting on their request.
File a Complaint
If you believe your privacy rights have been violated, you may file a complaint with Sprouts Health.
You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights.
Sprouts Health will not retaliate against you for filing a complaint.
HHS confirms that patients have the right to complain to their healthcare provider or to HHS about privacy concerns.
Your Choices
For certain types of health information, you may tell us your preferences about what we share.
Family, Friends, and Others Involved in Your Care
You may tell us whether we may share relevant health information with family members, friends, or others involved in your care or payment for your care.
If you are unable to tell us your preference, we may share information when permitted by law and when we determine that doing so is in your best interest.
Marketing
We generally must obtain your written authorization before using or disclosing your protected health information for marketing when HIPAA requires authorization.
Sale of Health Information
We will not sell your protected health information without your written authorization when authorization is required by law.
Psychotherapy Notes
If Sprouts Health maintains psychotherapy notes, most uses and disclosures of those notes require written authorization except as otherwise permitted by law.
How We May Use and Share Your Health Information
We typically use or disclose your health information in the following ways.
Treatment
We may use and share your health information to provide, coordinate, or manage your healthcare.
For example, we may communicate with laboratories, pharmacies, specialists, or other healthcare professionals involved in your care.
Healthcare Operations
We may use and share your health information to operate Sprouts Health and improve the quality of care.
This may include quality improvement, healthcare administration, training, credentialing, compliance activities, and business operations.
Payment
We may use and share health information when necessary to obtain payment for healthcare services or coordinate payment-related activities.
Because Sprouts Health operates as a cash-pay practice, payment-related disclosures may be more limited than at insurance-billing practices, but permitted disclosures may still occur when applicable.
Business Associates
We may share protected health information with companies that perform services on behalf of Sprouts Health, such as electronic health record providers, secure communication platforms, billing or payment vendors, technology vendors, or other contractors.
When required by HIPAA, these entities are subject to agreements requiring them to protect your health information.
Other Uses and Disclosures Permitted or Required by Law
We may use or disclose your health information without your written authorization in certain situations permitted or required by law, including:
- public health and safety activities
- reporting suspected abuse, neglect, or domestic violence when required or permitted
- health oversight activities
- responding to certain judicial or administrative proceedings
- responding to law enforcement requests when permitted
- workers’ compensation matters
- organ and tissue donation
- medical examiner or funeral director activities
- preventing or reducing a serious and imminent threat to health or safety
- research when applicable legal requirements are satisfied
- compliance with federal or state law
HHS includes these categories among the standard permitted uses and disclosures described in a provider NPP.
Substance Use Disorder Records
To the extent Sprouts Health creates, receives, or maintains substance use disorder patient records that are protected by 42 CFR Part 2, additional federal confidentiality protections may apply.
Such records generally may not be used or disclosed in investigations or legal proceedings against a patient unless permitted by applicable law, such as with appropriate written consent or a qualifying court order.
As of February 16, 2026, HIPAA-covered providers must include information addressing Part 2-protected substance use disorder records in their Notice of Privacy Practices.
Our Responsibilities
Sprouts Health is required by law to maintain the privacy and security of your protected health information.
We will notify you if a breach occurs that may have compromised the privacy or security of your information when notification is required by law.
We must follow the privacy practices described in the Notice currently in effect.
We will not use or share your health information for purposes other than those described in this Notice unless you give us written authorization or another law permits or requires the use or disclosure.
If you give us written authorization, you may revoke it in writing at any time, subject to actions already taken in reliance on that authorization.
HHS requires covered providers to describe these legal duties in their NPP.
Changes to This Notice
Sprouts Health may change the terms of this Notice.
Changes may apply to all health information maintained by Sprouts Health, including information created or received before the change.
When material changes are made, an updated Notice will be made available on SproutsHealth.com and as otherwise required by law.
