Could Low Testosterone Be Affecting How You Feel?

Testosterone deficiency may contribute to changes in sexual health, energy, and body composition. Symptoms can overlap with other medical conditions, so proper evaluation matters.

Low Libido

Reduced sexual desire or changes in sexual function.

Persistent Fatigue

Low energy that doesn’t improve with adequate rest.

Muscle & Strength Changes

Loss of muscle mass, strength, or physical capacity.

Body Composition Changes

Increased body fat or difficulty maintaining muscle.

Mood & Motivation Changes

Reduced motivation or changes in mood and well-being.

Reduced Morning Erections

Changes in spontaneous erections or sexual health.

These symptoms don’t automatically mean you need testosterone therapy. Sleep apnea, thyroid dysfunction, medications, and metabolic conditions can produce similar concerns.

More Than a Testosterone Number

Total testosterone is important, but it may not provide the entire clinical picture. My approach considers symptoms, medical history, repeat morning testosterone testing when appropriate, and additional markers that help clarify hormone status.

Individualized Hormone Evaluation

  • Total testosterone
  • Free testosterone when clinically indicated
  • SHBG when relevant
  • Other hormone and health markers as needed
  • Sleep, metabolic health, and lifestyle factors

Thoughtful Treatment Decisions

Treatment decisions are based on more than a single laboratory value.

I consider symptoms, confirmed biochemical deficiency, medical history, potential benefits, risks, and alternatives.

The goal is appropriate treatment, not chasing an arbitrary testosterone target.

Personalized Testosterone Replacement Therapy

For men with symptoms and confirmed testosterone deficiency, testosterone replacement therapy (TRT) may be an appropriate option. Treatment requires individualized assessment and ongoing monitoring.

Potential Benefits

  • Improvement in sexual desire
  • Improvement in some sexual symptoms
  • Increased lean body mass in some men
  • Improvement in certain deficiency-related symptoms

Results vary, and improvements in energy, mood, and physical performance are not guaranteed.

Safety & Monitoring

  • Testosterone levels and treatment response
  • Hemoglobin and hematocrit
  • Blood pressure and relevant health risks
  • Prostate assessment when appropriate
  • Sleep apnea and fertility considerations
  • Dose adjustments when clinically indicated

Frequently Asked Questions

Do I need low testosterone to qualify for TRT?

Testosterone therapy is generally considered for men who have compatible symptoms and consistently low testosterone levels confirmed through appropriate testing. A single borderline result is not enough to make a treatment decision.

What if my total testosterone is normal?

Symptoms deserve evaluation even when total testosterone is within the laboratory reference range. Depending on the clinical situation, free testosterone, SHBG, and other potential causes of symptoms may need to be assessed. Normal results do not automatically mean TRT is indicated.

Can testosterone therapy affect fertility?

Yes. Exogenous testosterone can suppress sperm production and may substantially reduce fertility. Men who are planning to have children should discuss this before starting treatment.

How often will I need bloodwork?

Testing frequency depends on the medication, dosing schedule, treatment response, and safety considerations. Laboratory monitoring is generally more frequent during treatment initiation and after dose changes, with periodic reassessment once treatment is stable.
